Simple Buttonhole -- HELP!?

Guys,

Better to cast off, leaving the yarn on the left and then immediately cast on?

or

Better to cast off, knit to the end of the row, then backwards loop cast on?

Other methods?

Just making a simple neck warmer and want to put a buttonhole or two in it near the end...

I was never very satisfied

....I do a knitted cast on for the second side....
